Home Health Care With A Conscience

Synergy Home Health Systems is dedicated to providing skilled medical services in the comforts of the client's home since 2010. As some home health care companies are clinic based, or corporate based, we consider ourselves community based. We have been educating patients directly about rights, fraud, abuse, and empowering them with choices, since 2010. When others force patient's to services they don't need, Synergy Empowered them with the options they have. Our experience range from treating clients who may be in need of recovery from an acute episode of illness, rehabilitation for injuries, recent hospitalization, or nursing home stay. Our goal is to aid patients in returning to their prior level of function, educate in managing their current health status, and maintain the ability to keep patient's safe in their home.

Our assessment is very thorough. We assess the overall and complete situation of the client's need, from clinical, functional , services, financial, and environmental factors that may pose as challenges. Once we gather all the information, we generate a care plan which involves their physician, Synergy staff, and outside resources if necessary. Throughout this process, the patient/caregiver/family member will be informed of what we are trying to achieve.

When our care is complete, the staff will ensure that all necessary things a patient needs is still ongoing, such as diabetic supplies, community resources, medical equipment etc. A follow up call is always made well after home health care is discharged just to make sure everything is going well, and if there is anything further we can provide.
